Pierre Pigray 1532-1613, surgeon-in-ordinary to King Charles IX, King Henri III, and King Henri IV. This work was first published in 1609 and appeared in 43 editions in French and Latin in the course of the 17th century. WorldCat only appears to identify one copy of this edition (from the same year, in Lyon, with the portrait on the title page and the same number of pages, though with a different printer).
